It has become necessary for students who wish to shadow physicians
while they are practicing in hospitals to receive additional
approvals and process additional paper work. Beginning immediately,
the Office of Student Services will require the following for
students seeking to shadow physicians in hospitals:
1. Have all of your immunizations up-to-date and bring proof
from the Immunization Data Base to the Office of Student Services
in order to receive a "Directed Studies Form." You
will not be issued a "Directed Studies Form" unless
all immunizations are up-to-date.
2. Obtain and complete (except for Dr. Falls' signature) a "Directed
Studies Form." You will need to obtain signatures of the
physician with whom you are going to shadow and the Chairperson
of the appropriate clinical department. The "Directed Studies
Form" needs to have the dates you wish to shadow the physician
in the hospital.
3. Obtain a "Permission to Shadow a Physician in the Hospital
Form" from the Office of Student Services. This form should
be taken to Dr. Falls who will complete the top half of the form.
The form along with the "Directed Studies Form" should
then be taken to the Medical Education Office at the hospital
in which you wish to shadow a physician. Have the appropriate
representative in that office sign the form giving you permission
to shadow in the hospital on the date(s) indicated. The hospital
should keep the original form and copies should be returned to
the Office of Student Services .
4. After all paperwork is completed and approved, return the
signed "Directed Studies Form" and the signed copies
of the "Permission to Shadow a Physician in the Hospital
Form" to the Office of Student Services. If all forms are
signed and approved, Dr. Falls will sign the "Directed Studies
Form" allowing you to register for the shadowing experience.
