The Patient-Centered Medical Home Presentation Series

In March of 2007, the American Academy of Family Physicians, the American Academy of Pediatrics, the American College of Physicians and the American Osteopathic Association, endorsed the “Joint Principles of the Patient-Centered Medical Home”.  Blue Cross Blue Shield of Michigan, as part of their Physician Group Incentive Program, has developed a reimbursement mechanism for practices which implement tools and components of the patient-centered medical home.  2008 PC-MH initiatives include reimbursement for individual care management, extended access, test tracking, patient registries, performance reporting and patient-provider agreements. 

HealthTeam will host a series of presentations on each of the initiatives, including examples of how HealthTeam clinics have already successfully initiated implementation.  Presentations will be held the first Tuesday of every month, from 5:30 to 7:00 p.m., in the Radiology Auditorium beginning March 4. 

The Department of Family Medicine will host two Grand Rounds dedicated to the patient-centered medical home.  On March 26, David Walsworth, M.D. will present “The Patient-Centered Medical Home as Part of Improving Performance in Practice”.  On November 26, David Walsworth, M.D. and Rebecca Malouin, Ph.D., M.P.H. will present “The Patient-Centered Medical Home: Definition, Research and Implementation”.  Both presentations will take place in the Sparrow Professional Building Conference Room E from 8:00-9:00 am.
